STUDENTS <br />3004 <br />Page 3 of 4 <br /> <br />Bonneville Joint School District No. 93 <br />Immunization Requirements <br />2) A statement indicating that the student is exempt from immunization as <br />provided in Section 110 of this rule for religious or other objections; and <br /> <br />3) The signature of the parent, custodian, or legal guardian. <br /> <br />Exclusion from School <br /> <br />1. The Bonneville Joint School District No. 93 will deny attendance to any student in <br />preschool (pre-K), or kindergarten through grade twelve (K-12) not in compliance <br />with this policy. <br /> <br />2. Any student denied attendance will not be allowed to attend any schools w ithin the <br />District until he/she is in compliance with this policy. <br /> <br />3. Students who are diagnosed or suspected of having a contagious or infectious <br />disease or students who have been exposed to a contagious or infectious disease <br />for which they have not been immunized will be excluded from school. <br /> <br />4. The district will also close school on order of the state board of health or the local <br />health authorities. <br /> <br />Reporting Requirements <br /> <br />A copy of a report of the immunization status in required grades will be submitted to the <br />Department of Health and Welfare on or before November 1 of each year. <br /> <br />DEFINITIONS: <br /> <br />Conditional Admission to be eligible for conditional attendance, a child must have <br />received at least one dose of each required vaccine and currently be on schedule for <br />subsequent immunizations. <br /> <br />Exemption is the process of freeing oneself from a rule or an obligation imposed on <br />others. <br /> <br />Medical Exemption is when a child has a medical condition that prevents them from <br />receiving a vaccine. <br /> <br />Nonmedical Exemptions is for religious or philosophical reasons. <br /> <br />Adopted: 02-11-2004 Reviewed: Revised: 06-08-2005 <br /> 04-12-2017 <br /> 05-13-2020 <br /> 12-09-2020 <br /> 12-14-2022 <br />
